License Plate Form

Keiser University has partnered with the Indian River County Tax Collector’s Office to enable Seahawks from ANY COUNTY IN FLORIDA to be able to purchase their KU license plate through the mail (and at no extra fee). All you need to do is submit the following form and a representative will contact you and walk you through the process of ordering your plate and having it delivered in the mail. That’s right – no trip to the DMV required!

Upon submitting this form, a representative from the Indian River Tax Collectors Office will contact you during regular business hours. Please be prepared to provide:

• Vehicle info (make, model, tag info)
• Mailing address
• Payment Information
• Insurance Information
  

The following fees will be charged in addition to your regular registration fees:

• $30 Specialty plate fee ($25 of which goes to support Keiser University student scholarships)
• $28 One-Time plate replacement fee (if within 3 months of your birthday)
